You are enrolling in, or are responsible for enrolling a student in, the adult class at Actor’s Edge and by submitting this form you understand and agree to the following:
12 class initial enrollment period for first time students: There is a 12 class minimum enrollment period for first time students. If this enrollment is for a first time Actor’s Edge adult class student you agree to make payments for at least 12 classes (3 months worth of class) regardless of your attendance. Return students do not have a 12 week minimum enrollment period.
A $99 deposit is required to enroll. Your $99 deposit will be applied toward your tuition.
Class tuition is $175 per month. (oftentimes we offer discounts if you pay your full remaining balance on the first day of class – email us for current discounts at firstname.lastname@example.org)
Payment methods accepted: All forms of payment are accepted (check, credit card, paypal, cash, etc).
AUTO PAY DEFAULT: if payments are not made by their due dates in another form, your credit card used to submit your enrollment deposit (or a replacement credit card on file) will automatically be charged for any balance due.
NEW STUDENTS payment schedule: After paying the enrollment deposit, remaining tuition for the 3 month minimum enrollment period may be paid in full at your first class (ask about discounts with this option) or paid in monthly installments. If paying monthly, payments (usually $142 each) are due on your start date, four Sundays after your start date, and eight Sundays after your start date, regardless of your attendance. (you can get up to two weeks credits for missed classes if you have advanced notice (see terms below)).
Joining Ongoing Class: After your three month minimum enrollment period, if you choose to continue in class as an “ongoing” student, payments are due on the first class of each month regardless of your attendance, until you terminate enrollment. If your three month minimum enrollment ends mid month and you choose to stay in class, your first payment as an ongoing student will be due on the following sunday and will be pro-rated based on how many classes remain in the month. Tuition will be pro-rated on a class by class basis during any month in which there are less than 4 classes.
RETURNING STUDENTS/ONGOING CLASS payment schedule:
If you are a returning student, your first month’s payment is due on your start date and will be pro-rated based on how many classes remain in that month. After that, payment is due at the first class of every month regardless of your attendance. Tuition will be pro-rated on a class by class basis during any month in which there are less than 4 classes.
LATE PAYMENT FEE: if payments are not made by their due dates and the credit card on file will not process payment, a $10 per week late fee will automatically be added for up to four weeks.
Credit for missed classes: You may get credit for up to two missed classes in any three month period if you provide 30 days advanced written notice about the classes you will miss, or, if the absences fall within the first 30 days of your class, written notice about those dates upon enrollment.
Terminating Enrollment – 30 DAYS NOTICE REQUIRED (if in ongoing class): Once you have completed your 3 month minimum enrollment period, if you choose to stay in class as an “ongoing” student, (or if you are a return student enrolling in the ongoing class), 30 days written notice must be emailed to email@example.com to terminate enrollment.
If you fail to provide written notice 30 days prior to terminating enrollment in the class, you are authorizing Actor’s Edge to immediately charge your credit card a pro rated amount of your tuition for the number of classes unpaid for during the 30 days following the date of written notice, or following your final class, (whichever is earlier). If you are still within your initial enrollment period, you will also be charged for any additional classes that fall within your initial enrollment period.
Working Credit Card on File: You agree to keep a working credit card on file. A $1 test charge will be charged and refunded to any new credit card number delivered to Actor’s Edge. If, for any reason, your credit card will not process the test charge, or charge on due dates, you agree to the following: a) if you are in ongoing class, you will immediately submit a “final month” payment at your normal tuition rate. This payment can be applied to your final month in class after you deliver 30 days notice of termination. AND you will pay a $10 per week late fee on any payments not made by their due dates regardless of your attendance on those dates. b) if you are in your initial enrollment period, you will submit payment in another form for the remainder of your balance due for your enrollment period AND a $10 per week late fee on any payments not made by their due dates regardless of your attendance on those dates.
Actor’s Edge IS A TALENT TRAINING SERVICE. THIS IS NOT A TALENT AGENCY CONTRACT. ONLY A TALENT AGENT LICENSED PURSUANT TO SECTION 1700.5 OF THE LABOR CODE MAY ENGAGE IN THE OCCUPATION OF PROCURING, OFFERING, PROMISING, OR ATTEMPTING TO PROCURE EMPLOYMENT OR ENGAGEMENTS FOR AN ARTIST. Actor’s Edge IS PROHIBITED BY LAW FROM OFFERING OR ATTEMPTING TO OBTAIN AUDITIONS OR EMPLOYMENT FOR YOU. IT MAY ONLY PROVIDE YOU WITH TRAINING. FOR MORE INFORMATION, CONSULT CHAPTER 4.5 (COMMENCING WITH SECTION 1701) OF PART 6 OF DIVISION 2 OF THE LABOR CODE. A DISPUTE ARISING OUT OF THE PERFORMANCE OF THE CONTRACT BY THE TALENT SERVICE THAT IS NOT RESOLVED TO THE SATISFACTION OF THE ARTIST SHOULD BE REFERRED TO A LOCAL CONSUMER AFFAIRS DEPARTMENT OR LOCAL LAW ENFORCEMENT, AS APPROPRIATE.
Your right to cancel: You may cancel this contract and obtain a full refund, without any penalty or obligation, if notice of cancellation is given, in writing, within 10 business days from the enrollment date or the date on which you commence utilizing the services under the contract, whichever is longer. For purposes of this section, business days are Monday through Friday. To cancel this contract, send the following notification to: firstname.lastname@example.org
Cancellation Notice: “I hereby cancel this contract. Dated: Artist Signature.” If you cancel, all fees you have paid must be refunded to you within 10 business days after delivery of the cancellation notice to the talent service.
All material written, created, or performed by students for this class, and any product thereof, becomes the property of the Actor’s Edge. Actor’s Edge reserves the right to use all material created or performed in class, as well as student’s face and likeness, at its own discretion, including for marketing, promotional, or commercial purposes.
Right to change terms: Actor’s Edge reserves the right to change the terms of this agreement at anytime by providing written notice. Actor’s Edge will make every effort to send written notice at least 30 days before changing any terms in this agreement.
Parent or legal guardian must enroll student under 18 years of age.
If a student under the age of 18 is enrolling, by checking the box below, agreeing with these terms and conditions, you are also certifying that you are the parent or legal guardian of the student enrolling and have read and agreed to these terms.
